ECU provides a range of forms to assist students in managing their academic journey. Whether you’re planning to take a leave of absence, declare a minor or request official documentation, the following forms are available to support your needs.

OneCard Replacement Request
If you’ve lost, damaged, or misplaced your OneCard, you’ll find instructions for what to do on our OneCard page.
Change of Address Form
A form is not needed. Current students can update their addresses through MYEC. But if you require a form:
Verification of Enrolment Form
If you would like official proof of your enrolment status, please fill out the form below and email it to reghelp@ecuad.ca. Please allow 3–5 business days for processing.
Declare a Minor Form
Students wishing to declare a minor must complete this form and submit it to Academic Advising.
Information Release Authorization Form
To authorize the university to share your academic information with a third party, please complete this form and submit it to the appropriate area via your Emily Carr University email. This permission will be kept on file for your studies unless otherwise notified in writing.
Leaves of Absence
Undergraduate students who need to take a break from their studies can apply for a leave of absence for up to one year. This guarantees a space in your program upon return. If you’re leaving mid-semester, you must also submit a Withdrawal from ALL Fall or Spring Course Form. Email all completed forms to Academic Advising.
Graduate students should contact the Graduate Studies office for information on leave of absence policies and procedures.
Letter of Permission
Students may request a Letter of Permission (LOP) for studies they plan to do at other post-secondary institutions for transfer back to Emily Carr University. To guarantee that the coursework will be accepted for transfer, the student must request approval before registering for the course(s) and allow four to six weeks for processing. Please get in touch with transfercredit@ecuad.ca for more information.
Withdrawal Forms
Undergraduate students must email an official withdrawal form to reghelp@ecuad.ca to withdraw from a course or the university.
Graduate students should contact the Graduate Studies Office for withdrawal policies and procedures information.
Fall and Spring Semesters

Summer Semester
Order a Replacement Degree Parchment
Alumni from undergraduate and graduate programs can request a replacement degree parchment by filling out a Degree Parchment Replacement Request Form and submitting it in person, by fax or by mail.
We do not accept requests by phone or email. The cost is $50.00. Processing takes six to eight weeks.
